"Donor Value Statement"

Does anyone currently incorporate a “donor value statement”, or something similar, into planned giving agreements? It is my understanding that this is a type of statement in which the donor provides their intent for how the fund will be used once it is funded upon the donor’s passing. Also, is anyone requesting a brief bio from their planned giving donors? Something they would like to share with their future scholars?

Any information, advice, and/or examples you can share would be most appreciated. Please omit any confidential information.
