Cycle Debrief / User Survey

We are wrapping up our first university-wide application cycle. We would like to debrief with our scholarship administrators and faculty reviewers. Has anyone used a survey to gather feedback from reviewers and opportunity admins that you would be willing to share an example of?

We would also be interested in a cycle debrief/user survey if a client institution has one to share. We drafted one to use last year, but it ended up on the backburner while we focused on other issues. If no one else has one, we would be glad to share with you what we drafted.
