Designing a form for applications in Forms Manager

Hi it's me again. I'm  in Forms Manager designing our online application for grants and have added the attachment page. I have one box for attachments and need five. I want the applicant to be able to load the attachments separately and to use a table with all of the names of the required documents e.g. List of Board Members, Project Budget, Operating Budget, Audit, Tax Exempt Letter.


How do I do this?
