I need to run a report that shows everyone who donated using a particular ETap Online Form. How do I do that?

This is going to depend entirely on how you are tracking the donations that come in from that form. Are they for a particular Campaign? Do they all have a unique UDF marked? There isn't a way to query on a specific form without first knowing how you track that data. If there is nothing unique about that DIY form from other ones you have live, then we cannot distinguish between them. If you know what makes these donations unique, we can query on that field.
