Adding a Document to a To-Do Activity

I want to add a document to a To-Do activity that is not a request requirement or a payment requirement. For example, to assign a new activity to a request record, I go the the Related Activities and choose add a new activity. However, there is no where in the window to add a document to the activity or to add a document even when selecting the Related Documents for this new activity.

Any assistance would be appreciated.
