Changes for OAuth User Consent Form

We elevated the level of consent on the OAuth user consent form from "product - tenant" to "environment" to simplify the authorization experience for developers and users. The recent deprecation of tenant_id and tenant_name in the OAuth /token response enabled this to be a non-breaking change, and the OAuth user consent form has been updated. The change will not affect applications that already have valid access tokens.
