404 Error on query calls in Power Automate

Question

In the past week, I've been intermittently getting 404: Resource Not Found when executing an ad hoc query call in Power Automate.  The flow is triggered by a tile in RE, and when the run fails, if the user submits the tile again, it works the second time.  Any ideas what's going on and how to fix it?

Finally getting around to working through this 404 error too. Unfortunately I'm needing to switch out every "Send an HTTP request" with "Start an ad hoc query" in all of my flows, which hasn't been fun, but it seems to be working. So thanks to all who figured this out!

Confirming, was using the sky add-ins connector.  Will get it switched out asap.  Thank you @Erik Leaver and @Alex Wong  for being awesome!

It appears that when multiple parameters are sent via the send as http request in the SKY Add-in Connector there is an extra ampersand being added inconsistently.
