Bulk Import Constituent Sample Error

In attempting to use Bulk Import, we would like to import Constituent Religion information. We were happy to find within the constituent sample at the very bottom an entity mapping to "Custom - ConstituentReligion". Unfortunately, we have found that this sample has an error. If you attempt to copy this sample as is and paste it as a mapping for an import process you receive the error "The entity type 'Custom - ConstituentReligion' does not exist." Two questions:

1. What is the correct mapping to insert and/or update religion codes on constituents?
2. How can you use the Custom table entity?
   1. Can we get a sample mapping?
