Adding $5 extra option to forms

Does anyone know how to give donors the option to add an admin fee on top of their donation? You know, the "$5 ask." "Yes! I'd like to cover the administrative fees so my entire gift goes the field. OR YES! I'll give an extra $5 to an emergency relief fund.
