Upload Immunization Forms - Nurse

What are other schools or nurses doing when you have to have an actual immunization form.  You would think there would be a place to upload an actual immunization form and have it show on a students medical record.  


Any thoughts or work arounds

    The only way I have found is to upload it into conditions.  I added a new condition that is blank. (so it doesn't show red on student's profile) and upload the record there.  I then restrict access so I am the only one that can see it.  

    I did the same thing with exemption forms.  But I named the condition E.  (I don't want to call it exemption because it will show up on the student contact form.). Student's with exemption forms will show up as a red E, but I can still restrict who can see it.  If a teacher is curious ("what does this red E mean?"), I just say that is for my record keeping and doesn't relate to anything medical in the classroom.
