For those of you using the online recommendation forms what are you using for a release of records form?

We have a pdf form attached to the checklist step parents can download, sign and bring to their child's school. 

If anyone has a more streamlined process, I would love to hear about it.

Same here. Ideally, would it be possible to create an online Release of Records form? Parent would esign and it would be sent from the portal to the current school.
