Using webforms for COVID contact tracing/vax status?

Is anyone using webforms as a way to pre-register and collect contract tracing/vax status. Since we are a part of a university, we are required to collect re-registrations for contact tracing as well as a self-attestation that the visitor has received the vaccine. I'd love to know how others are doing something similar! The biggest issue with the webforms is that you can't make the Yes/No checkbox required, so I have created a dropdown “Yes/No” question but it doesn't stop someone from submitting a registration form if they select “no”. This then becomes difficult to monitor, if someone selects “no”.

Would love to know what others are doing! Thanks very much!

    Hi Jill, I understand your university's requirements, but I wanted to let you know that after conferring with our security and legal teams, we do not recommend asking and tracking vaccine status in Altru, as that is personal health data. That being said, I do have a solution for your yes/no dropdown problem--only enter Yes as an option, then No will not be an option they can select.
