Class page > Topic: Cannot add image to Topic

In a Class Page > Topic tab, I created a Topic; went to Setup > Edit Settings > Cover Photo > Change Photo. I clicked “Browse”, found a JPEG and clicked Open. Nothing changed; the image was not added.

The image file is a few 100 KB.

I also tried with a smaller GIF: same result.

I was able to add an image to another topic, a few weeks ago:

I had a similar issue last week in two different places. One was uploading a jpeg to a student's user file and the other was with the custom report card. It took about 5-10 tries each time before it would finally upload.
