Can I create a general contact form with fields of my choosing?

We want to create a form on our website that collects information from interested donors and volunteers on how they want to participate in our organization. We want to collect address information, as well as some attribute or note fields with specific questions. Is this possible within RE NXT or with an outside app that can integrate with RE NXT? I'm trying to avoid just creating a google form that has to be entered into our database. THANK YOU for any advice!

You could do this by re-purposing an NXT or OLX Event Form. It will not be able to do everything you want, for instance the answers to additional questions would appear within Participant Options or Participant Attributes, respectfully. If you are okay with an additional extra work within the database this could be a solution. If you would rather have a form that does everything you are looking to do, I would recommend running it through a third party app; this option would likely involve manual entry to importing the results into your system.

Blackbaud WAS working on this about two years ago. I remember it very distinctly and I believe they were possibly even in Discovery or EAP. Then it vanished. You'll have to find something else unfortunately.
