Is there a way to add a suffix field to go along with title, first name, and last name?

@Matthew Smith - Not directly, however donors with a suffix will frequently include this information within the Credit Card Holder Name field. You could also add a gift attribute to the form to collect the donor's preferred Donor Recognition, as indirect way of collecting this information as well.
