can I remove something that seems locked in the form settings?

I am using Emergency Phone Contact block, but I do not want two of the questions which appear to be locked.

Is this phone active and Emergency contact phone type. They are shown as Required Yes, locked and Visibility: Show locked.

Is there a way to remove these?
