Event Registration Form Not populating field in RE

We have an Event Registration form for a free event. If someone registers that is not already in our database as a donor, RE creates a record but it does not populate the addressee/salutation fields that typically are required fields. Am I missing something in the registration form configuration setup?

database view's required field is not enforced in webview.

There is no functionality right now in the event registration form to specify default addressee/salutation for newly created record.
