Pledge Notification - Adaptive Card/SkyAddIn

I created an adaptive card that when pledge notification is selected from the drop down add-ins menu, a Microsoft form is launched that a team member completes and the answers are sent a share point spreadsheet. Since the form is launched from a constituent page, is it possible to pass the const id, const name to the form or spreadsheet? I’m trying to simplify how much info the team needs to complete. Also, the the final stage of this process is that I’d like a pledge automatically added to the const record. Is that possible? Any guidance is much appreciated!

what you trying to achieve cannot be done with MS form, or any existing form sky add-in allow.

you will need to create adaptive card where the submission is handled through the adaptive card completely.
