Why some check boxes disappear from donation form?

We have a donation form that was used last year. This year I found two check boxes disappear:

Anyone knows. Any thoughts will be help.

Are you editing the default css files?


I think you are missing some code in the default.css


I don't see this id that file: #content input[type="radio"]

If I get rid of the appearance style from customstyle.css at line 69 they display. Not really clear to me WHY that is happening though.
