Embed photo uploader?

Does anyone know of a tool we could use for the following scenario?

We are asking donors to fill out a survey on our website that is essentially their testimonial as a donor.
We need them to sign a release form, for this - I can just drop in a PDF link for them to download on the thank you page after they submit their survey answers.
But do you know of anything we can embed on our page that will let the donor upload the file once it is downloaded and signed and/or a photo?

I know there is an upload option in the personal fundraising, but is there anything outside of that? Maybe a third party that is free or affordable?

Hey Amanda,


We have used SurveyMonkey. They have the ability to upload files via their software.


It's just another survey question.


I can't remember if the free version allows for that, we have the paid version.


I hope that at least gives you an option.

Thanks! Are their surveys embeddable on pagebuilder sites? Or do you just use them separately?

I am pretty sure they are able to be embedded. We have not done that yet because I have only used the file upload survey one time and I just added our logo to it.


We use LO surveys for almost everything.
