How do you request bank information from your grantees?

COVID-19 has become a catalyst for our Foundation to move to EFT and ACH payments vs. checks for our grantees. We are creating a form to request the needed bank information while at the same time looking at various ways to insure our grantees' privacy. I would like to know how other Foundations have set up this part of their operation, including getting acknowledgments that the grantee has received the payment.  Thanks in advance.
