Template for Colleagues to Request Queries or Reports?

Hello!


Does anyone use a template from which colleagues can request queries or reports? I'm trying to create something that requests certain parameters, but wanted to see if anyone had any ideas.

  • JoAnn, that looks great. Is that a form included with your software? I wonder if anyone is using an Outlook form. A colleague suggested trying that to replace the NetCommunity ones we are using right now. We have separate data request forms for :

    website updates

    events

    research

    fund creation

    appeal creation

    email blasts

    financial info

    Mark Guncheon‍ not 100% sure what you mean by 'included with your software.' The form was created by a predecessor. Any forms submitted, come by email to me landing in a specific inbox.
  • If you are using Wordpress for your website design, there are certain form plugins you can use to create forms for your pages. NetCommuntiy has the "Common form" which is what we are currently using to create forms. The submissions through that form are directed to email addresses of those responsible for each data request.

  • Margi, this looks fantastic. It's enough to help whoever is handling the request but not too much to overwhelm the person making the request.
  • Thanks, Mark! It has been a tremendous help to us as well as the requesters. I should have added that we have the due date field set to where it has to be at least two weeks out. Despite noting this in two different places on the form, we were getting "surprised" with lots of last-minute requests. They still happen now and then, but at least this forces people to discuss things with us first, so we can assess their needs and plan our schedule accordingly. :)
  • Our due date is a minimum of five days. Typically, my requests are done sooner but occasionally take longer. We were also getting those "I'm headed to a meeting in an hour and need a list of all donors for the past five years who have donated through family foundations but only on weekdays."
  • What powers this? i.e. is this a Drupal Form? and how do you manage these requests as tickets?

  • I recently found JotForm. This one looks great! How do you manage these requests once submitted? Can you correspond with the requestor in a way that documents all of the work/action taken?

  • Yes, its jotform, which is extremely easy to use. We used Cherwell to create tickets previously and now we use JIRA, which is free for up to 10 users. If you have Outlook email, when the email notification comes in, a ticket can be created directly from the email with the Jira add-on. The add-on also allows you to send emails directly to the Issue (Jira's term for a ticket). I hope this helps. Thanks for your feedback, also.
