Report is pulling unwanted text from application form

Hi, community! When I run a report of one of my saved searches, some of the fields are auto-populating with the text from the corresponding field in the application form. I don't know how this is happening and I want to stop it from happening. 


For example, there is a field on one of our application forms that says: "Please list any additional branding that is included in your sponsorship. If no other branding is included, simply type none." When I run the coordinating report, my "other sponsorship benefits" column includes the applicant's input to this field but it ALSO includes the exact text from the application form ("please list any additional branding..."). 


Another similar example: I have a field on this application form called "mailing address." When I run the report, in the column I intended, I see the applicant's response to this field (their mailing address), but I also see the words "mailing address." Again, it is pulling the verbiage directly from the application form and inserting it into the report. 


I am stumped and can't figure out what it is about these 2 fields in particular that is causing this to happen! This is not happening for any of the other inputs that are present in the report.
