Athletics Forms

@Michaela Murphy we use a sports preference form for students to request what sport for each season.

@Michaela Murphy Yes, we use BB Forms for all athletic forms. I have created about 12 different forms that we need each season from participants. However, we only accept hard copy athletic physical forms, and I think a lot of schools do them electronically, and they have issue with trying to do that in BB - so I don't have that problem. I use Event registration for the parents to purchase sport specific practice gear, like shirts and pants, since that is size specific and some kids want 3 shirts and some want 1, etc.

@Michaela Murphy we use BB forms - for physicals and other physician signed forms we use a Medical school form (Academics) because that form allows you to upload to the Nurses Office - it's been great moving to all digital and parents don't have to come on campus to drop forms off - no lost paperwork ?

@Ravendra Persaud the parent scans the form that the physician signed (Physical) and uploads on the Medical type form that we title “Athletic Physical." This way the upload is maintained by the Nurse and there is no paperwork to get lost.

We do the same thing for team sign ups - use the “very dated” Custom Forms. We do use Magnus for physician records but the Nurse handles that.
