Emergency Contact Info - How do you collect it?

How do you collect emergency contact information from families? I don't see a form with that field option.

I did create a custom field for emergency contacts; however, it does not appear to update the Emergency contacts on the contact card and therefore does not pull into any reports. I'm thinking about field trips, etc.

Which leads me to another thought - what report(s) do you use for field trips to provide teachers with what the information they may need to access quickly?

  • @Jennifer Miller When you create a school form, you choose between, I think 5, different form types. One of them is “emergency contact.” As far as I know, that's the only way to get a form that populates those specific fields.

    We print the medical contact card for field trips etc. Teachers can print them from their class roster. There is also an emergency contact report, under reports.
