Let's start with, what is the donation link (since it's public donation form, it should be ok to post here as it isn't sensitive) so we can see how the donation form is structured

when you say “description and gift option”, what does that mean? how is that information being gathered on the donation form

“we list 4 items”, is that “Designations”? (meaning it is fund that can be selected) or is it a Custom field being used to ask “custom question” on the form?

“The report only shows” what report are you using?

I see what you mean, you simply change the “label” of the dollar value. This will not show up in RE NXT anywhere.

What you are trying to do is more like “selling a product/service”, that is more in the territory of eshop/ecommerce.

The closest thing I can think of that will better satisfy this is using RE NXT Event Registration where you can setup “other fee” (non registration fee), which will allow donor to “buy” X number of something. (i.e. 5 $30 Mercy Graduate Personalized Lawn Sign).

As far as “reporting”, you will need to report through event participant list, which depending on your purpose for reporting, may be good enough. However, in the gift record that is created, you are not going to see what they purchased, UNFORUNATELY. This is something I am advocating Blackbaud to fix. While the event registration (and other) fee can be setup with Campaign, Fund, Appeal, Package default, it is not granular enough. I am advocating Blackbaud to allow Campaign, Fund, Appeal, Package designation for EACH Fee Level. This way you will be able to tell from Gift level what was “purchased”
