Workflow Not Pulling One Constituent That Falls Within the Parameters

I have a workflow that would add an action to a constituent if they donate $1,000-$2,999 to a specific fund. This has worked without a hitch so far, but we just found out it didn't add an action to one constituent last week. I'm trying to figure out why, but I couldn't seem to find a reason. I checked the flow runs from the day the gift was added and the next day, and it's not on either of them. However, another gift added on the same day was pulled. I even made a workflow including the constituent's System Record ID to run manually, but it's still not pulling the constituent.

Am I missing something? This is very odd. Below is the workflow steps.

first thing to check is if the gift was soft credited, which will affect if the action will be added to the "right record" or not

2nd thing is provide screenshot for the full List gift action, as it is currently not showing all the filters you are using.

Thank you, Alex. The gift does have a soft credit, but the action should only be created in the hard-credit donor record, as the List Gift action only pulls the hard-credit constituent ID, correct?
