Private Evalation type and IEP date on Candidate Profiles

There is a question on applications, "Does the candidate have a private evaluation?", with 3 options available: Educational, Learning Style, and Speech. There is also a question for asking if they have an IEP, and then what the IEP date is.

On a candidate profile, I see checkboxes for whether they have an IEP and private evaluation, but no fields for evaluation type/date or IEP date.

I recall seeing the type/dates on other profiles, so not sure if it's something with this application. Any ideas?
