Hi,


I get requests from colleagues asking for a data pull.  For example, I want anyone who has given over the last 2 years.  Then I have to contact them asking exactly what the criteria is, omits, fields needed etc.  I was wondering if anyone had a form that they request be completed before running a data pull and if you would be willing to share.

    Having them write down their request is a great idea.  It forces them to really think about what they want before they ask!

    I set up this Google Form at a previous job and it worked really well. I recommend Google Forms as they're super easy to set up and feed each submission into a Google Sheet, so you have a good log of data requests.


    This form covered all the basics, although you may want to tailor some things to the needs of your organization. I rarely had to go back to someone asking to clarify after they filled out this form.


    The other benefit to the form that I hadn't considered when I created it was that it made people think ahead. With the form I instituted a strict two-business-day turnaround policy, and I ended up getting most requests at least a week before they were actually needed. VERY few last-minute requests. This helped the whole department immeasurably.

    Our institution has a MachForm contract, and so i built both of these forms for our department.


    One is internal development department use (use a bogus email to get through.  it won't submit anything ot me until YOU hit submit.  Check it out as you see fit!).

    The other is for external campus partners.


    I still am looking for more and new ways to handle these. but maybe it's helpful to you!
