Custom School Forms

Hello- we are new to Blackbaud and in implementation.  As a Toddler-8th grade school, our preschool campus has required forms by the State of California.  I'm having trouble figuring out how to build these is school forms.  I'm wondering how other CA schools work dealing with the Identification and Emergency Information Lic700 and Lic 601, Physician's Report Lic 701, Child's Pre-admissions Health History LIC 702, Personal Rights LIC 613 A, Parents' Rights LIC 995?

    Is it possible to upload some examples of your forms here? Also to confirm - you want to build the content of those forms within School Forms, not have them upload completed versions of the actual state forms, right? I deal with custom forms a lot and am happy to help but I'm also need a visual :)

  • Our local licensing agency has never allowed for these forms to be electronically signed so we have the parents print them out and mail (or drop them off) back to us. 


    We group the forms into several "Health Packet" PDFs based on status and grade level (New Preschool/Pre-K, Returning Pre-K, New K, Returning K, New 1st-8th grade, etc..). Those pdfs are included as a link in separate School Forms, again based on the same divisions as the packets. These School Forms include a simple "Yes I have printed and returned the forms" checkbox to turn off the reminders. 


    Once we receive the signed paper forms, we track them using "Admissions immunizations requirements" in the Nurse's Office.
