Asking for Race/Ethnicity on application

I am wondering if anyone asks applicants for race/ethnicity on their application? I had a place for race on our application last year and included a comment that it was not required so families understood that it was not something we used in our admission decision. However, because of the statistics we have to report every year to DASL (Data and Analysis for School Leadership) it would be great to have families enter how the student identifies.

If you have it on your school's application, what wording do you use? Do you ask for both race and ethnicity? Does your school only ask for this information when enrolling, if at all?

No, we do not require it

We require ethnicity and also require the Is Latino/Hispanic to be completed as well but on a different form once the student is enrolled.

Vicky, do you use a School Form to collect this information? Or another method?

We use a blank form that we created as a “Demographic Form” that is sent to parents when they formally enroll their child.
