Date Last Changed Not Updating - Telephone, Email

We've noticed when we mark a telephone number or email inactive or update a telephone number or email, the “Date Last Changed” column doesn't always update with the date of the change. The problem is sporadic; we haven't been able to piece together what's causing the issue. We have tried tab through and just checking the inactive box. The outcomes are inconsistent.
