Medical Forms for new Candidates

Does anyone have an idea of what is best practice to collect and handle medical information for new candidates and not violating HIPPA

  • @Soudad Albermani Are you able to hold off on collecting medical forms until enrollment? I've used both the in-system Nurse's Office as well as 3rd party services like Magnus Health in the past, but only with users with the Incoming Student role, not Candidates.

  • @Jess Moxley We have not started the enrollment process just yet, I was thinking if there is a way to collect health forms safely and directed straight to the Nurse to check then we get her feedback if there is any missing document needed.

  • @Soudad Albermani We use all the health forms made available through Blackbaud, but only for students, not candidates. We have them as part of the Student Checklist to be completed at the beginning of each school year. All the forms are attached to the student in the Nurse's Office view. By using the forms available in the system, you can also run lists (reports) for allergies, etc. And it's all tied to the same core database of info…

  • @Soudad Albermani Does anyone have any thoughts on using Magnus Health vs. just using the Blackbaud medical forms? We use Magnus and I really don't like it - it is hard to use. But we switched over 5-6 years ago when the Medical forms were more limited, so I am curious how they are now.

  • @Soudad Albermani Like @Anita Pinkerton, we use the BBK12 forms. My understanding is that since we are a school - not an organization that delivers medical care - we are not legally bound by HIPPA (but check with your lawyer and/or advisor!). We ask candidates to submit immunization records and an abbreviated form for those participating in a shadow visit so the nurse knows to anticipate things that might arise. More robust medical reports are then required upon enrollment.

  • @Vickilyn Gill - You need to build the medical forms via Academics > Content > School forms > + Add Form > Medical.
