Learning Support Question

Hello Fellow Blackbaud Users!

Our Learning Support Dept. is looking for a way to digitize the Accommodations form that a student completes for testing accommodations, and then has their teachers and learning support teacher also complete as well before a test. I thought I had an option with Microsoft Forms because it looked like you could collaborate on a form, but looks like collaboration is only for the original form itself, not after someone completes the form.?

Would love to hear what others are doing in this case. Thank you in Advance!
