Visiting Student Evaluation

Hello!

We are looking for a way to send an internal evaluation form to our teachers who may have interacted with a student on their visit day. Does anyone have a good way of doing this through Blackbaud? It was suggested to use a Google Form, but then I would have to save each submission as a PDF and upload under misc. files for the student (with 6+ potential submissions per student).
